Promoting healthier lifestyles to reduce chronic diseases and health disparities

The Centers for Disease Control and Prevention (CDC) awarded Prince George’s County Health Department (PGCHD) a Community Transformation Grant (CTG) to support PGCHD’s efforts to improve the health and wellness of its residents.

The CTG initiative, called Healthy Eating, Active Living (HEAL), set out to promote healthier lifestyles and to reduce chronic diseases and health disparities in the following 12 zip code target areas:

10 Reasons to shop at the Farmers’ Markets in your community

  2. WIC cards, Senior Fruit & Vegetable Checks (FVC) and Senior Farmers’ Market Nutrition Program (FMNP) Checks are accepted at most farmers markets.
